the resulting data. For those who order 100 or more questionnaires,
the following products are included:
With your order, you receive:
- Questionnaire forms (you decide the quantity);
- Instructions on how to administer the questionnaire;
- A tabular report as a pdf file on CD
- A printed executive summary (sample)
of findings based on the machine-scored form and computerized
data analyses (student surveys only)
- For grades 6-12, the executive summary is tied to reporting
requirements of No Child Left Behind.
- Reports come with a free chapter on the core measures
required by the Drug Free Communities Support Program.

Multi-Year Comparison Report $325 (Available
for Grades 4-6, Grades 6-2, Risk and Protective Factor, and Communities
that Care surveys)
For schools that have conducted the Pride Surveys student questionnaires
for two or more years. The Multi-Year Comparison Report uses color
charts to compare data from surveys conducted in your school system
over several years.
Multi-media Presentation $275 (Available
for Grades 4-6, Grades 6-2, Risk and Protective Factor, and Communities
that Care surveys)
This PowerPoint-like presentation is the perfect tool for sharing
your data with school boards, parent groups, community coalitions,
the news media, and any other group. Showing the community that
you are taking a science-based approach to solving adolescent problems
is a great way to engender public support.
Raw data $375 (Available
for all surveys)
Raw data is available in ASCII format on CD with record layouts
for those who wish to conduct additional analysis of their data.
You will need our own statistical analysis software, such as SPSS.
Data Mining $125 (Available
for all surveys)
Examine subgroups within your survey population. You determine the
variables. Example: Respondents who associate high risk with marijuana
use. This is ideal for sites that do not purchase Raw Data.
